Vermont has received a Rural Health Transformation grant of $195 million in Year One of a five-year opportunity aimed at building stronger rural health networks, improving technology and shared operations, and strengthening the rural health workforce. The grant focuses on improving practice and administrative entity capacity to calculate clinical measures and track community health team services. By enhancing data and measurement capacity, practices will be able to provide high-quality health care services and strengthen data-driven care coordination and performance improvement. The application deadline is 4pm on July 3, 2026.
