This Request for Proposals (RFP) seeks qualified offerors to assess and plan training for Frontline Health Professionals in chronic disease prevention and management. The scope includes assessments and planning for CHW/CHR Chronic Disease Education, Workforce Development, Communication Strategy, Training Program Expansion, Colonoscopy Access, and Residency Training Development for Colonoscopy/Polypectomy.
