STATEMENT OF WORK AND GENERAL SPECIFICATIONS For Driving Simulator

General Information The Rehabilitation and Extended Care Line is requesting to procure one Driving Simulator for use by the Physical Medicine & Rehabilitation Driver s Rehab outpatient clinic. The location of the clinic is at the Micheal E. DeBakey VA Medical Center campus. The Contractor must provide delivery of the system with the below specifications and features.

Salient Characteristics This is a request for a Driving Simulator System with adaptive controls and wheelchair access. The simulator must meet the following specifications and have the following operating features: Use of car parts for familiarity Three-point seat belt Instrument cluster Automatic shifter console Visual displays providing a 180-degree field of view (FOV) 55 display screens Ability to choose multiple driving scenarios Blind spot monitors Stereo sound to simulate traffic like environments Motion system that responds to driver s actions Adaptive hand controls to include Spinner knob Single pin Tri pin V grip Palm Grip Push-right angle (left and right side) Push-pull (left and right side) Push-rock (left and right side) Left-foot accelerator Removable driver seat module for wheelchair access 30 x 45 inch space for the wheelchair

Warranty and Support The warranty and support must cover the hardware and software of the complete system for a minimum of 2 years from purchase. Additional accessories and features installed (interchangeable grips and hand controls) should also have a standard manufacturer s warranty.

Training On-site training will be available to staff including Biomedical Technicians through vendor in-services. Training will occur during or after site installation but no longer than 5 calendar days after installation.

Delivery and Installation Location The Contractor will be responsible for delivery to: 2002 Holcombe Blvd Building 100, BB-100-100 WH NX TEAM Houston, TX 77030

The installation shall occur in the clinical space of 2B-272. Work will be completed Monday-Friday between the hours of 8:00 AM-4:30 PM.

Inspection/Acceptance Upon delivery the VA Driver Rehab Therapist or Point of Contact will perform an inspection of the equipment in the presence of the contractor personnel, prior to the contractor personnel final departure of the VA facility. This inspection will ensure that all equipment is accounted for in the delivery and that there is no discrepancy with the delivery manifest. Any damaged items identified during the inspection will not be accepted. The VA Driver Rehab Therapist or Point of Contact will inspect the functionality of the equipment and all adaptive features to ensure nothing is faulty.

Additional Services: Any additional items outside of this Statement of Work are not authorized, unless approved through a modification to the contract by the CO. No one else has the authority to obligate funds for the VA.
