Description

The Pasadena Outreach Response Team (PORT) is a multidisciplinary, field-based program designed to engage individuals experiencing homelessness who face complex barriers related to mental health conditions, substance use, chronic medical needs, and justice system involvement. PORT prioritizes low-barrier, trauma-informed, and person-centered engagement strategies to stabilize participants and connect them to housing, healthcare, and supportive services. The City of Pasadena Public Health Department seeks to contract with a qualified nonprofit agency to provide peer-led outreach and engagement services to strengthen participant engagement and continuity of care.
