Description

The South Dakota Department of Social Services, Division of Behavioral Health via the Department of Health is requesting applications to support transformational behavioral health system improvements, including development, expansion, or sustainability planning for the Certified Community Behavioral Health Clinic (CCBHC) model of care. Eligible applicants are limited to organizations selected for the 2026 CCBHC Planning Grant Cohort. The grant supports activities across provider capacity building, crisis services expansion, IT infrastructure development, and workforce development to meet SAMHSA's CCBHC criteria and improve rural behavioral health services. Multiple awards will be granted with no match requirement, funded through the Rural Health Transformation initiative supported by CMS.
