Description

The Embassy invites you to submit a quotation for JUSMAG SASP & Contractual Employees Health Insurance under RFQ NO. 19RP3826Q0071.  

NOTES:

The offeror must submit a notarized copy of its most current license, certificate, or accreditation issued by the Government of the Philippines or its authorized agent (e.g., Insurance Commission, relevant board), confirming that the offeror is duly authorized to provide health insurance coverage in the Philippines. Failure to demonstrate that the offeror is an authorized insurance company permitted to write and administer health insurance policies in the Philippines shall be grounds for rejection of the proposal. The U.S. government does not sign any documents or agreement outside of the organization for the confirmation of order. The issuance of a fully approved and signed purchase order, by the Contracting Officer, from the U.S. government will serve as the confirmation of acquiring the items and/or services.
