36C24826Q0711 Page 1 of Performance Work Statement WPB_ ARMORED CARS SERVICE 1. Introduction The Government requires Armored Car Services to pick up/drop-off all collected monies from the West Palm Beach VA Medical Center's Agent Cashier and Veterans Canteen Service (VCS) to the chosen financial institution. The services will also include the delivery of change orders (coin currency) to the Agent Cashier and Veterans Canteen Service, in support of the West Palm Beach VA Healthcare System. 2. Description/Scope/Objective The Contractor shall provide all labor, equipment, tools, materials, supervision, and other items and services to perform the work as defined in the Performance Work Statement. The Contractor shall pick up deposits and drop off change orders three times per week at the West Palm Beach VA Medical Center, for both the Agent Cashier and the Veterans Canteen Service. The West Palm Beach VA Medical Center is located at 7305 North Military Trail, West Palm Beach, Florida 33410. The Contractor personnel shall always display a picture ID badge while on Medical Center grounds and shall take all precautions necessary to protect staff and patients while performing armored services. 3. Specific Tasks The service locations include the Canteen Service Office on the first floor in Room 1C-237 and the Agent Cashier's office in Room 1B-119 at the VA Medical Center, 7305 N. Military Trail, West Palm Beach, FL 33410. Services shall be provided every Monday, Wednesday, and Friday unless otherwise directed by the Contracting Officer for the duration of the contract period. The banking institution for this contract is: Wells Fargo Bank 4480 Northlake Blvd Palm Beach Gardens, FL 33410 Phone: 561-838-5644 Banking The Contractor shall drop off and pick up deposits at the above bank location. They shall also pick up and deliver any money orders, change orders, or supplies from the bank to the appropriate locations (either Agent Cashier or VCS). Delivery times for change drop-off shall be between 8:30 AM and 10:30 AM on designated days or as directed by the Contracting Officer and/or the Contracting Officer's Representative (COR). Pickup of cash deposits shall also be between 8:30 AM and 10:30 AM on the specified days or as directed. Contractor personnel will take all necessary safety precautions during transportation. While inside the medical facility, they shall maintain high awareness of safety and privacy for patients, visitors, and staff. Licensure The contractor shall be licensed under Chapter 493 by the Florida Department of Agriculture and Consumer Services and shall provide proof of licensure to the contracting officer within 7 days of contract award. Insurance The contractor shall maintain an Armored Car All Risk Cargo Transportation and Storage Insurance Policy from a licensed insurance company in Florida and the contractor's home state, rated A- or better by A.M. Best. The policy shall include coverage for Theft, Disappearance, Destruction, Forgery, Employee Dishonesty, and Property other than Money and Securities, with a minimum of 120 days to file a proof of loss. It shall also include temporary employees in the definition of employees. A certificate of insurance shall be provided before service begins. The contractor shall assume liability if insurance is canceled or lost. The Commercial General Liability policy shall be maintained with a minimum of $1,000,000 per occurrence and $3,000,000 in total, naming the VA Medical Center as additional insured. The policy shall include coverage for Products and Completed Operations. The Commercial Automobile Liability shall have a minimum of $1,000,000 Combined Single Limit. Other liabilities include insurance for deposits and change funds, covering coin, currency, and checks up to $50,000 per occurrence, and reconstruction of checks up to $25,000 per occurrence. The contractor shall also have motor carrier liability as required by Florida law. Damage or loss reports shall be furnished within 36 hours, and the agency shall assist in recovery efforts. 8. Performance Monitoring POC monitors contractor performance by verifying with Cashier and Canteen that work was completed as per the contract. Routine weekly communication is maintained for verification. 9. Security Requirements VA Form 2280 submitted to ISO. 10.
