Description

The State of Delaware Department of Health and Social Services is seeking professional services for the enhancement and maintenance of the Cancer and Chronic Disease Data Management System (CCDDMS), formerly known as the Screening for Life (SFL) system. This system supports the Division of Public Health's response to statewide strategies aimed at mitigating the impact of chronic disease risk factors related to cancer, hypertension, diabetes, asthma, and other chronic conditions.
