The Department of Defense (DoD), through the Office of the Under Secretary of Defense for Personnel and Readiness (OUSD(P&R)), is seeking public comment on a proposed information collection initiative: - Government Buyer: - Department of Defense (DoD) - Office of the Under Secretary of Defense for Personnel and Readiness (OUSD(P&R)) - No Original Equipment Manufacturers (OEMs) or commercial vendors are involved, as this is not a procurement of products or services - Products/Services Requested: - Information collection for Military Community and Family Policy Records Requests (DD Forms 3216, 32161, 3217) - Application for DoD Impact Aid for Children with Severe Disabilities (SD Form 816) - Status of Forces Survey of Active Component Members (annual survey) - Unique/Notable Requirements: - Compliance with the Privacy Act and Freedom of Information Act - Use of secure electronic portals for submission - Standardized data collection for records access, impact aid applications, and annual surveys - Place of performance and contracting office: - Department of Defense offices in Alexandria, VA and Washington, DC

Description

The Department of Defense (DoD) has submitted a proposal for collection of information under the provisions of the Paperwork Reduction Act. This is a 30-day notice for information collection. The notice informs the public of the DoD's intent to collect information and invites comments to be submitted by June 22, 2026. The collection aims to standardize data collection for Military Community and Family Policy records access requests and other related information collections.
