Description

The Indian Health Service (IHS) is seeking applications for the Phase 2 Produce Prescription Pilot Program (P4). This program supports Tribes, Tribal organizations, and Urban Indian organizations in implementing produce prescription models that increase access to nutritious and community relevant foods and promote whole-person health and well-being. Award recipients will establish or expand programs that connect health care providers with food systems to provide fruits, vegetables, and traditional foods to eligible participants. Programs are expected to integrate clinical services with community-based food access strategies and community appropriate nutrition education that reflect community values and practices. Recipients will develop partnerships with health care providers, community organizations, and food suppliers to implement sustainable produce prescription models, prioritizing populations experiencing food insecurity and chronic conditions influenced by food access and nutrition such as diabetes.
