The Hawai'i County Office of Aging (HCOA) is requesting proposals from qualified applicants to provide Case Management Services. The project aims to assist older adults experiencing diminished functioning capacities or other conditions by providing access to services and care coordination through compensated service providers or family caregivers. Additionally, Family Caregiver Case Management will offer support services to family caregivers to help them maintain their caregiving abilities.
