(current base plan provider) - Zurich American Insurance Company (current catastrophic plan provider) - Student Assurance Services (claims administrator) - The Brokerage Store (claims administrator) - Products/Services Requested: - Group Student Blanket Accident Insurance (Ameritas, Policy #42-26-4752-814-848-5) - Maximum benefit: $25,000 per injury, no deductible - Covers medical expenses, accidental death and dismemberment, and related medical services - Catastrophic Blanket Accident Insurance (Zurich, Policy #MCB 7934150) - Includes $500,000 cash benefit - Two plan options: - District-Paid Plan: Basic and catastrophic coverage mirroring current policies - Voluntary Parent-Paid Plan: Covers UIL activities and/or combined accident insurance for school time and/or 24-hour coverage - Detailed coverage categories for both plan options: - In-patient benefits, out-patient surgery, other out-patient benefits, physician services, motor vehicle injury, other benefits (e.g., heat stroke), field trip coverage, accidental death and dismemberment, catastrophic coverage - Claims administration services - Unique/Notable Requirements: - Proposals must duplicate current coverage or clearly identify alternative plans as 'Alternative Proposals' - Compliance with federal, state, and local insurance laws, including ADA - Minimum insurance coverage limits, AM Best rating, and additional insured endorsements required - Detailed coverage provisions, compliance documents, and certifications must be submitted - Reporting of renewal rates and loss run data 90 days prior to contract anniversary - No price increases in the first year; subsequent years may negotiate upward price changes - 120-day notice required for cancellation or non-renewal - Evaluation based on coverage, cost, service, and vendor qualifications - Period of Performance: - Initial contract term is 12 months, with up to four one-year renewal options (total up to 60 months) - Annual renewal cycle aligned to district academic and athletic schedules - Place of Performance/Delivery: - United ISD facilities in Laredo, TX, including Purchasing Department Warehouse, Shipping and Receiving Warehouse, and Accounting Central Office
