The platform aims to reduce avoidable Emergency Department utilization and preventable inpatient admissions and readmissions across its Medical Access Program (MAP), MAP Basic, Sendero Health Plans, and affiliated safety-net populations. Respondents may submit qualifications reflecting their expertise and capabilities, including specialized portions of the services.
