Description
Combine Synopsis/Solicitation # HT940626QE032 is issued as a Request for Quote (RFQ). The combined synopsis/solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Regulation as indicated in the attachment. The DD Form 1449 is being utilized to generate applicable clauses.
The Defense Health Agency, Healthcare Contracting Division- Southeast intends to procure a firm-fixed price contract for Medical and Pharmaceutical Waste Removal and Disposal at Naval Health Clinic Cherry Point, NC. The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform Medical and Pharmaceutical Waste Removal and Disposal as defined in this Performance Work Statement (PWS) except for those items specified as government furnished property and services. The contractor shall perform to the standards in this PWS. The contractor shall comply with all federal, state, and local laws and regulations for removal, transportation and disposal of medical and pharmaceutical wastes. This requirement is being solicited in accordance with Federal Acquisition Regulation FAR Part 12.6; Streamlined Procedures for Evaluation and Solicitation for Commercial Products and Commercial Services.

Submission of Quotes: Offerors must follow the requirements of FAR 52.212-1 Addendum, 52.212-2 Addendum, and the Quote Instructions and Evaluation Criterion provided in the solicitation document to create the quote. Quote is required to be submitted electronically to the Contracting Officer and Contract Specialist on or before the date and time specified in the RFQ.
