Johnson VA Medical Center in Charleston, SC. - Government Buyer: - Department of Veterans Affairs, VISN 7 Network Contracting Activity, Hatcher Building - Suite 2, 501 Greene Street, Augusta, GA 30901 - OEMs and Vendors: - PHARMALINK, LLC (referenced as the Master Service Agreement holder and service provider) - Products/Services Requested: - Pharmaceutical reverse distribution services for non-salable pharmaceutical products - Includes processing, shipment to manufacturers for credit, disposal of non-returnable products - Detailed return inventory reports and certificates of destruction - Access to web-based inventory and reporting applications - On-site service: PharmaLink field representatives inventory and package items at the facility; priced at 10% of Estimated Return Value (ERV) - Off-site service: Customer inventories and packages returns; PharmaLink arranges pickup and shipping - Inventoried disposal: Hazardous and non-hazardous waste pharmaceutical disposal at specified per-pound rates ($2.50/lb general, $0.99/lb hazardous/Schedule II-V) - C2 Services: Management of DEA 222 forms and affiliated paperwork - Unique or Notable Requirements: - Compliance with all federal, state, and local regulations for pharmaceutical returns and disposal - Services must provide detailed inventory and destruction documentation - Both on-site and off-site service options available - Emphasis on secure handling and regulatory compliance for controlled substances - Place of Performance: - Ralph H. Description

This statement of work (“SOW”), adopts and incorporates by reference the terms and conditions of the Master Service Agreement (“Agreement”) which was entered into on March 24, 2026 between PHARMALINK, LLC, a Delaware Limited Liability Company, with its principal place of business located at 8285 Bryan Dairy Road, #200, Largo, FL 33777 ("PLI") and VAMC Charleston place of business located at 109 Bee St. Charleston, SC 29401 collectively “Customer”. Landing together with PLI, the "Parties", and each a "Party", as it may be amended from time to time.  This SOW shall run commensurate with the term of the Agreement.  Transactions performed under this SOW will be conducted in accordance with and be subject to the terms and conditions of this SOW and the Agreement.
