The Department of Veterans Affairs is seeking vendors to provide community-based, short-term residential services for homeless Veterans under the Health Care for Homeless Veterans (HCHV) Contract Emergency Residential Services (CERS) Program in San Francisco. - Government Buyer: - Department of Veterans Affairs - San Francisco Veterans Affairs Health Care System - Network Contracting Office (NCO) 21, 261-Network Contract Office 21, Mather, CA - OEMs and Vendors: - No specific Original Equipment Manufacturers (OEMs) or named vendors are identified; the opportunity is open to qualified service providers. - Products/Services Requested: - Community-based, short-term residential services for up to 9 homeless Veterans at a time - Emergency housing, case management, mental health and substance use disorder treatment, vocational assistance, and support for transition to permanent housing - Services to be provided in San Francisco, preferably in the Bayview-Hunters Point neighborhood - Year-round availability of 9 beds (approximately 3,285 bed-days per year), for up to five years (five 12-month periods) - Unique or Notable Requirements: - Contractor must provide a residential facility in San Francisco, with preference for Bayview-Hunters Point - Services must include comprehensive support: housing, case management, mental health, substance use disorder treatment, and vocational assistance - The program is designed to support stabilization and transition to permanent housing for homeless Veterans - The contract may run for up to five years, with annual ordering periods - Period of Performance: - Up to five years, with five 12-month ordering periods - Place of Performance: - City and County of San Francisco, CA (preferably Bayview-Hunters Point) - San Francisco VA Medical Center and VA Downtown Clinic are referenced as government facilities
