Description

This requirement is to provide on-site SCBA Physicals & Spirometry Testing on as need basis at a local medical facility, and on site visit from the medical provider to Susanville, CA 96130 for the Lassen National Forest.  SEE Attachments 1, 2, 3, and 4 for more detailed information.  The documents are included as attachment to this solicitation and can be accessed via the "Attachment/links" section of this posting.
