The USDA Forest Service, Lassen National Forest, is seeking a contractor to provide on-site SCBA (Self-Contained Breathing Apparatus) physicals and spirometry testing for 79 employees in Susanville, California. - Government Buyer: - USDA Forest Service, Lassen National Forest (Region 5, Northern California Acquisition Service Area) - Products/Services Requested: - On-site SCBA physical examinations and health screenings for 79 employees - Spirometry testing for 79 employees - Mileage reimbursement for travel related to testing - Annual and as-needed medical evaluations and written recommendations on employee fitness for SCBA use - Requirements: - All testing must comply with OSHA 29 CFR 1910.134(e)(1) and ANSI Z88.6-2006 standards - Services must be performed within 300 miles of Susanville, CA - Contractor personnel must have USDA LincPass for facility access - Compliance with federal anti-discrimination laws is required - No specific OEMs or vendors are named in the solicitation - The opportunity is a total small business set-aside under NAICS 621340

Description

Requirement is to provide on-site SCBA Physicals and Spirometry Testing on as needed basis at a local medical facility, and on-site visit from the medical provider to Susanville, CA 96130.  SEE Attachments for detailed information.
