The Wyoming Army National Guard is seeking a contractor to provide comprehensive Technician Surveillance Exams and occupational health services for approximately 70 federal technicians at the Army Aviation Support Facility (AASF) in Cheyenne, Wyoming. - Government Buyer: - Wyoming Army National Guard, under the Wyoming Military Department and U.S. Army National Guard - Contracting office: W7N9 USPFO ACTIVITY WY ARNG, Safety and Occupational Health Office - OEMs and Vendors: - No specific OEMs or commercial vendors are named in the solicitation or attachments - Products/Services Requested: - Medical Surveillance Physicals and Technician Surveillance Exams for about 70 federal technicians annually - Services include: - Physical and vision exams - Chem 20 lab tests, blood lead and heavy metals testing - Pulmonary function tests and respiratory fit testing - Dermatological exams and job function analysis - Detailed reporting, record keeping, and compliance with DoD and OSHA regulations - All labor, materials, equipment, portable sanitation, insurance, permits, transportation, and tools - Services must be performed by certified physicians or physician assistants - Unique or Notable Requirements: - Compliance with DoD Instruction 6055.5, DoD Instruction 6055.5-M, and OSHA 29 CFR 1910 - Coordination for access to F.E. Warren Air Force Base - Strict privacy, security, and quality control standards - Annual reporting of contractor labor hours - Contract includes a base year and five one-year option periods - Place of Performance: - Army Aviation Support Facility (AASF), Cheyenne, Wyoming - F.E. Warren Air Force Base, Cheyenne, Wyoming - No specific products, part numbers, or OEM equipment are required; the focus is on professional medical and occupational health services

Description

The contractor is responsible for all costs and shall provide all labor, materials, equipment, utilities, portable sanitation facilities,insurance, permits, transportation, tools and parts necessary to perform TECHNICIAN SURVEILLANCE EXAMS at the AASF in Cheyenne, Wyoming as stated IAW the attached Performance Work Statement.
