Eagle Healthcare Greensboro, LLC - Nurse Aid, LLC dba Angel Hands - Personal Care, Inc. - Shipman Family Home Care, Inc. - NC Personal Care Services, LLC - Services Requested: - In-Home Aide Services Level I (Home Management for adults with IADL impairments) - In-Home Aide Services Level II (Home Management and Personal Care for adults with partial ADL/IADL dependency) - In-Home Aide Services Level III (Extensive Personal Care and Home Management for adults with significant ADL impairments or medically unstable conditions) - In-home aide supervision and services (including client care per plan, supervision visits, competency verification, infection control, and personnel record maintenance) - Notable Requirements: - Providers must comply with North Carolina regulations for supervision, personnel qualifications, and service delivery - Certified Nursing Assistants (CNA) and Registered Nurse (RN) supervision required for Level III - Quarterly and annual supervisory visits mandated, with increased frequency for higher care levels - Providers must demonstrate competency in personal care tasks, maintain documentation, and adhere to infection control and health screening (e.g., TB tests) - MWBE participation goal of 10% and E-Verify affidavits required - Service areas must be geographically limited to ensure effective supervision - No OEMs or product part numbers are applicable, as this is a service-based procurement - Funding sources include HCCBG and SSBG grants
