Synopsis/Solicitation updates delivery location information for federal healthcare facilities in Oregon. The delivery location for this requirement is Warm Springs Health & Wellness Center, 1270 Kot-Num Road, Warm Springs, OR 97761. Quoters should ensure the capability to provide services and/ or deliver equipment to this location; failure to do so may render the quote non-responsive. All other terms and conditions remain unchanged.
This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

Title: Warm Springs Health & Wellness Center- Optical Coherence Tomography with Table and Angiography Installation, Calibration, and On-Site Training Services.
Location: Warm Springs Health & Wellness Center, 1270 Kot-Num Road, Warm Springs, OR 977761
Scope of Work: Procure, deliver, remove existing and disposal, install, and verify operation of a Maestro2 Optical Coherence Tomography with Angiography (OCTA) system with table at Warm Springs Health & Wellness Center. Includes software and upgrades (if Applicable), removal and disposal of existing OCTA equipment, installation, calibration, warranty, documentation, 24/7 support (if applicable), on-site training, optional service contract coverage, and HER/PACS integration. Includes compliance with manufacturer specifications, safety and HIPAA requirements, and all labor, material, and equipment, and transportation necessary for full delivery and installation.
Performance Period: Date of award through one year of contract. The performance period includes delivery, removal of existing equipment, installation, testing, and final acceptance. Warranty coverage and support services shall continue for one (1) year from date of final acceptance.
