The Agency for Healthcare Research and Quality (AHRQ), on behalf of the Secretary of Health and Human Services (HHS), has delisted the symplr PSO (Patient Safety Organization) following its voluntary relinquishment of PSO status. This action is authorized under the Patient Safety and Quality Improvement Act of 2005 and the Patient Safety Rule. The delisting became effective at midnight ET on April 8, 2026.
