Description

The County of San Diego is seeking proposals from firms to provide in-home outreach and engagement services to individuals with Serious Mental Illness (SMI) who are reluctant to seek outpatient mental health services, as well as to their family members and caretakers. The services include in-home assessment, crisis intervention, short-term case management, peer and family support, psycho-education, and linkage to outpatient mental health care. The contract term is one year with four one-year options, and the anticipated contract maximum amount over five years is between $31,056,630 and $34,507,366.
