Description

The Florida Agency for Health Care Administration (AHCA) is issuing a Request for Applications (RFA) to establish grant agreements with eligible subrecipients under Florida's Rural Health Transformation Program (RHTP). This federally funded five-year initiative aims to improve rural health through five strategic goals and includes 14 initiatives. The Preventive Care and Care-at-Home Bundle (Bundle One) creates a coordinated continuum of rural preventive care and care-at-home services, including mobile health units, community paramedicine, remote telemonitoring, retail pharmacy clinics, and data infrastructure via HIE/ENS. All five initiatives within the bundle are required and must be delivered together as an integrated program.
