The Department of Veterans Affairs, Network Contracting Office (NCO) 21, is seeking information from potential vendors for providing licensed residential care facility services for homeless Veterans in Northern California. - Government Buyer: - Department of Veterans Affairs (VA) - Veterans Health Administration - Network Contracting Office 21 (NCO 21) - Northern California VA Health Care System - Services Requested: - Residential Care Facility for the Elderly (RCFE) services for homeless Veterans - 1 to 5 beds required (starting with 4 beds) - Services include: - 24-hour supervision - Assistance with Activities of Daily Living (ADL) and Independent Activities of Daily Living (IADL) - Case management - Room and board (private or shared bathrooms, common areas) - Meals and transportation - Supportive services to facilitate transition to permanent housing - Facility Requirements: - Must be a licensed Adult and Senior Care Facility (RCFE) in California - Facility must have at least one year of establishment in specified zip codes - Located in Contra Costa, Solano, Yolo, Sacramento, or West Placer Counties - Quantities: - 1 to 5 beds (starting with 4 beds) - Estimated 1460 days of emergency housing services - Unique/Notable Requirements: - Facility must be licensed and established for at least one year in the required counties - Services are targeted for homeless Veterans eligible for VA health care - Length of stay per Veteran typically 30-90 days, with possible extensions - No specific OEMs or commercial vendors are named in the notice.

Description

Sources Sought Synopsis The purpose of this Sources Sought Synopsis is to assist in our decision-making process for a service procurement. This is not a solicitation, nor a request for quotes or proposals. Responses to this sources sought synopsis will be used by the Government to make appropriate acquisition decisions. The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award of a contract, nor does it restrict the Government to a particular acquisition approach. The Department of Veterans Affairs, Network Contracting Office (NCO) 21, is conducting market research to identify potential vendors with economic status of Service-Disabled Veteran-Owned, Veteran-Owned, and other Small Businesses that can provide the general requirements below. GENERAL REQUIREMENTS Contractor will provide residential care including care for Acts of Daily Living (ADL) and Independent Acts of Daily Living (IADL) care for Veterans and must have an Adult and Senior Care Facility license (Residential Care Facility for the Elderly RCFE ) in the State of CA to provide ADL and IADL care. The program will also provide case management services to homeless Veterans in order to assist them to improve their income, health care, and access to long term permanent housing. The program residential facility must be located in Contra Costa, Solano, Yolo, Sacramento and West Placer Counties in California (See PWS Addendum 1 Zip Codes). The program will provide each Veteran room & board, private or shared bathroom and common area. This contract will be for between 1 and 5 beds, starting with 4 beds, for men and women. The program should have a minimum establishment of one-year in the required zip codes.
