Description

This is a SOURCES SOUGHT and is for informational/market research purposes only. THIS IS NOT A REQUEST FOR PROPOSAL, QUOTATION OR BID, NOR A SYNOPSIS OF A PROPOSED CONTRACT ACTION. The purpose of this Sources Sought is to conduct market research to determine if responsible sources exist, to assist in determining the category of Small Business Set-Aside. The proposed North American Industry Classification System (NAICS) Code is 621340, Offices of Physical Therapists, which has a corresponding Size Standard of $12.5 million dollars.

The 125 Fighter Wing is seeking sources to perform injury prevention and mitigation, rehabilitation exercise services, and human performance optimization services to rated personnel assigned to the 125 Fighter Wing (125 WFW), Jacksonville, FL USA, as part of the Human Performance Optimization (HPO) Optimizing the Human Weapons System (OHWS) initiative. to provide and perform injury prevention and mitigation, pre-habilitation services, and human performance optimization services to rated personnel assigned to the applicable fighter aircraft units. The Contractor shall furnish consultation and reports, similar to the range of services found in a commercial facility, as well as services specialized to meet the needs of fighter aircrew. Services provided by the contractor shall include the following: initial assessment of each aircrew, care for personnel who individually seek the services of OHWS Specialist; subject matter expertise concerning pre-habilitation evaluation/assessment, injury prevention, accommodation processes and implementation, ergonomic assessment and implementation, remedial training, and optimized performance physical training (e.g. strength and conditioning for fighter aircrew). At the discretion of the government, the contractor shall provide the following resources under this contract: licensed Physical Therapist and/or licensed Athletic Trainer and/or licensed Strength and Conditioning coach and/or licensed Massage Therapist.These documents will NOT be shared at this time.

Responses to this sources sought notice must be submitted electronically (via email). Please return the attached Sources Sought Cover Sheet with your email response.  A contractor’s response to this Sources Sought shall be limited to the information sheet and five (5) additional pages of information for past performance or experience which may help in market research for industry capacity and shall include the following information:

1. Contractor’s name, address, point of contact, phone number, email address and CAGE code.

2. Contractor’s small business category and business size: Small Business, Small Disadvantaged, 8(a) firm, Historically underutilized Business Zones (HUBZone), Economically Disadvantaged Women-Owned Small Business (EDWOSB), and Service- Disabled Veteran-Owned Small business(SDVOSB).

3. Provide a maximum of two examples of projects similar in size, scope, and complexity with a brief description of each project, customer name, contract number, and the dollar value. Projects considered similar in scope to this project include a variety of field support requirements for the same projects simultaneously. 4. Provide additional relevant information on the contractor’s experience/capabilities as it pertains to the proposed work outlined.
