This is a Sources Sought Notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources, small and other than small businesses, to fulfil a potential requirement. Respondents are required to be registered and in good standing in The System for Award Management (SAM) https://www.sam.gov/portal/SAM/#1 Purpose and Objectives: Your responses to the information requested will assist the Government in determining the appropriate acquisition method.
NAICS code: 339113 and the size standard is 1000 EMP. NOTE: Respondent claiming SDVOSB and VOSB status must be registered and CVE verified in VetBiz Registry www.vetbiz.gov. Place of Performance: North Texas VA Health Care System VAMC, 4500 S Lancaster Road, Dallas, TX 75216 Anticipated period of performance: 30 Days ARO. Information sought: Response to this announcement shall not exceed 10 pages and should include the following information: (1) A tailored capability statement indicating the firm s ability to provide the services required in the Statement of work. The firm must include all certifications, qualifications, and training certificates to perform work on the outlined in the Performance Work Statement. Synopsis 1.0 OBJECTIVES This Firm Fixed Priced contract will be a base plus (4) option years for Neuromodulation therapy device and accessories. 2.1 DELIVERABLES Delivery of the Neuromodulation therapy devices and accessories will be completed on the 1st and 15th of each month. 2.2 SCHEDULE: The supplier shall deliver all items ordered under the contract during standard operating hours, Monday through Friday, excluding Federal Holidays, to the following location: Place Of Performance:

CPU Replenishment: Responsibilities: The contractor shall provide a monthly drop shipment to the CPU monthly. The contractor shall ensure appropriate stock is available to ensure prompt delivery can be made by the 15th of each month. CPU points of contact shall communicate with the contractor any issues concerning the shipment and delivery of products. Order Placement: The contractor shall deliver the requested quantities each month.

The supplier shall maintain sufficient stock and ship the requested quantities to the CPU on a bi-monthly basis. The requested amount shall be shipped to the CPU at no charge to the government or FOB destination. Fulfilling orders from people other than authorized individuals may result in loss or delay in payment for supplies/services provided under such orders.
