Description

The City of Davenport is seeking proposals from qualified vendors to provide onsite physical therapy services at its health clinic, the Total Wellness Center, as well as offsite physical therapy and employee wellness services within Davenport city limits. The contract term is initially two years with options for three additional one-year renewals. The scope includes clinical care, injury prevention, ergonomic assessments, wellness programs, and administrative compliance.
