Request for Information (RFI) - Overview This Request for Information (RFI) is issued solely for information and planning purposes only and does not constitute a solicitation. Submission of pricing, capabilities for planning purposes, and other market information is highly encouraged and permitted under this RFI in accordance with Federal Acquisition Regulation (FAR) 15.201(e). Attachment Details Portable RO Machines for Washington DC VAMC Brand Name Only Note: OEM distributor/authorization letter from the manufacturer is required for all responses. If no OEM letter is supplied, the vendor will be considered non-responsive and disqualified when the RFI closes. Other requirements include: Completion of Buy American-Foreign Trade Agreements-Israeli Trade Act Certificate (FAR 52.225-4). Answer all questions (1-14); failure to do so will result in disqualification. Purpose and Delivery Details This is for the U.S. Department of Veterans Affairs VA Medical Center (VAMC). Description The Washington DC VA VAMC requires delivery within 60 days from contract award to the following warehouse during normal business hours (8:00 a.m. to 3:30 p.m., Monday through Friday): U.S. Department of Veterans Affairs Washington DC VA Medical Center WAREHOUSE/ATTN: 50 Irving Street NW Washington, DC 20422. The U.S. Government is conducting market research to identify responsible sources capable of meeting the requirements. This may lead to an open discussion or a future solicitation. Submission of information is voluntary, and proprietary information should be marked accordingly. Questions to Address (1-14) Respondents capable of manufacturing these items should provide the following information for review: Business size status and representations (e.g., SDVOSB, VOSB, HUBZone, WOSB, Large Business). Indicate if your company is considered small under the NAICS code specified. Identify if you are the manufacturer or distributor of the items. Provide Manufacturer Name (OEM) & Manufacturer Part Number (MPN). Complete the Buy American-Foreign Trade Agreements-Israeli Trade Act Certificate. If not a manufacturer, provide OEM authorized distributor/reseller letter; absence results in disqualification. Confirm if all items are on FSS/GSA, and provide contract number, SIN, and OEM letter if applicable. Confirm if all items are on NAC/SAC or SEWP V, and provide contract number.
