1.      Statement of Work

2.      Acceptance Criteria List

3.      Sample Subcontract

4.       Rep Cert Form (required with submission if total Quote is in excess of $150K)

5.       Past Performance Reference Information Form (submit with Quote regardless of value)

*With your Quote submission, you must certify in writing that you meet and agree to all of the Mandatory Requirements outlined within Doc 0. RFQ AA-2026-05 and will manufacture per SOW.
