THIS IS A REQUEST FOR INFORMATION (RFI) SOURCES SOUGHT NOTICE
1. Responses to this Sources Sought must be in writing. The purpose of this Sources Sought Announcement is for market research purposes only, to make appropriate acquisition decisions and to gain knowledge of businesses interested and capable of providing the following services: The Department of Veterans Affairs Portland VA Medical Center located in Portland Oregon is seeking contractors to provide transitional housing services as part of its community-based Health Care for Homeless Veterans (HCHV) program. The HCHV Program, an essential and critical part of Veterans Healthcare Administration is vital for providing a gateway to VA Boise Healthcare System and community supportive services for eligible Veterans who are homeless. This emergency transitional housing program is designed to address the needs of the most vulnerable and at-risk homeless Veterans.
2. The NAICS for this requirement is 624221, Temporary Shelters. The product service code is G004, Social-Social Rehabilitation.
3. Interested and capable Contractors should respond to this notice by the date and time listed in this notice by providing the following via email only to james.harris13@va.gov. (a) company name (b) address (c) business size (e.g. large, small, other than small, Veteran Owned, SDVOSB) (d) phone and email of primary point of contact (e) SAM Unique Entity Identifier (UEI) number (f) A capability statement that addresses the organization s qualifications and ability to perform as a contractor specifically for the work described in the attached DRAFT statements of work attached below. Respondents are also encouraged to provide specific examples (e.g. contract number, point of contact information) of the Contractor s experience providing the same or similar services described in paragraph 4 below. (g) The capability statement shall address where the interested respondent(s) have provided these services in the vicinity of the VA Facility as required per the Statement of Work (see attached)
4. Required Services: The Veterans Integrated Service Network (VISN) 20 provides a full range of medical services, mental health services and therapeutic housing programs to Veterans in Portland Oregon within its catchment area. The Heath Care for Homeless Veterans program provides interim housing to those Veterans requiring an interim step while receiving intensive outpatient services; addressing issues relating to their homelessness, and while waiting to access permanent housing resources. 5. The Government is looking to Solicit and potentially award a Firm Fixed Price Indefinite Delivery Indefinite Quantity Contract with a five-year period of performance.
