This procurement seeks contractor-provided ambulance services for the Indiana Army National Guard at the Muscatatuck Urban Training Center (MUTC) in Butlerville, Indiana. - Government Buyer: - Indiana Army National Guard (Department of Defense, Department of the Army) - Managed by W7M7 USPFO Activity IN ARNG office, Indianapolis, IN - Supported by U.S. Army North (USARNORTH) - Services Requested: - Advanced Life Support (ALS) Ambulance Service - 2-person crew: 1 NREMT Paramedic, 1 NREMT EMT or A-EMT - Emergency response, patient transport, and field ambulance support - Contractor supplies vehicles, medical supplies, radios, and ensures communication with installation EMS and USARNORTH staff - Basic Life Support (BLS) Ambulance Service - 2-person crew: 1 NREMT Paramedic, 1 NREMT EMT or A-EMT - Same scope as ALS, but at BLS level - Quantities: - 1 ALS ambulance crew - 1 BLS ambulance crew - Unique Requirements: - All personnel must be licensed Paramedics and EMTs meeting Indiana and NREMT standards - Uniform and training requirements for all staff - On-site staffing required from 8:00 am to 4:00 pm each day - Contractor must provide all vehicles, equipment, and supplies - Place of Performance: - Muscatatuck Urban Training Center (MUTC), Butlerville, IN - BLDG 5144 training venue (approx. 1 mile from MUTC main entrance) - No specific OEMs or vendors are named; procurement is full and open with SBA set-aside

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart  12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Request for Quote; quotes are being requested and a written solicitation will not be issued.

1) The Indiana Army National Guard is soliciting W912L9-26-Q-9001 as an RFQ. 

2) This Firm Fixed Price (FFP) procurement is full and open. 

3) CLIN 0001 - Ambulatory Services

4)  A detailed description of the requirement can be located in the attached Purchase Description. 

5) Submission Requirements: A detailed description of work to be performed along with pricing information. Please read the PWS and accompanying attachment, titled, Evaluation of Commercial Items, carefully.
