Washington County Health and Human Services, Behavioral Health Division, Addictions Program, seeks to contract with a qualified provider to deliver Substance Use Disorder (SUD) Community-Based Outreach Services to Washington County residents discharging from the Emergency Department from July 1, 2027 to June 30, 2032. The services aim to reduce unnecessary Emergency Department admissions, coordinate post-discharge care, and improve access to community services and SUD treatment/recovery support. Providers must serve a minimum of 100 individuals each fiscal year and ensure services are client-centered, trauma-informed, and evidence-based. The contract terms are for each year with an option to renew for five years, subject to availability of funds.
