Description

BPO DDDP Temporary staffing for Certified Nurse Assistants (CNA), LPN's and/or RN's certified to practice in the State of Florida for their designated classification. AUTHORIZED USERS: DDDP Personnel. Vendor must provide documentation of a Level II Background Screening; as the vendor hires new employees this information must be provided throughout the term of the purchase order. RATE: PER ATTACHED RATE SHEET : RN @ $59.00/hour; LPN @ $48.00/hour; CNA @ $31.00/hour OVERTIME: Time & 1/2 in excess of 40 hours by single employee, for workweek. HOLIDAYS: Time & 1/2 (from 7:00 pm the day prior to 7:00 am the day after, except as noted 3:00) for New Year's Eve (3 pm), Independence Day, Christmas Eve (3 pm), New Year's Day, Martin Luther King Jr., Christmas Day, Easter, Labor Day, Memorial Day, Thanksgiving. CANCELLATIONS: If less than two hours notice is given, DDDP will be billed two hours at the normal billing rate. If SMP employee is confirmed for a shift and is cancelled upon arrival without notice, DDDP will be billed four hours at the normal billing rate. STAT CALLS: DDDP will be billed for the entire shift if the facility calls with less than two hour notice indicating that a position needs filling ASAP. If a SMP employee fails to arrive at the facility within two hours after confirmation of the shift, DDDP will only be billed for the time worked by the SMP employee. SMP EMPLOYEE PLACED IN CHARGE OF AREA: $4.00 additional per hour.
