Description

This contract is for Drug & Alcohol Testing Products & Services (including DOT and Non-DOT Testing) through a cooperative Minnesota Multistate Contracting Alliance for Pharmacy (MMCAP) participating addendum with the State of Iowa. All purchase orders issued by ordering entities within the jurisdiction of this MPA agreement shall include the MMCAP Contract #MMS2501110 and the State of Iowa Master Agreement #26357 on the order. Within Iowa, all state agencies, facilities, cities, counties, or education entities, or any entity funded in part with state tax dollars, are eligible purchasers and authorized to purchase products and services under the terms of the Participating Addendum instead of a separate competitive process. The parties agree to comply with the terms and conditions of the following attachments, which are incorporated by reference: Attachment 1: MMCAP Infuse Procurement Certification, Attachment 2: MMCAP Contract MMS2501110, Attachment 3: Signed Member-Requested Participation Addendum (MPA), Attachment 4: Price File.
