Description
This project aims to test whether integrating data analytics with Whole Health Coaching (WHC) and Peer Specialists (PS) can reduce the reliance of homeless Veterans on acute care services. The study involves a randomized controlled trial (RCT) with 220 Veterans, comparing usual care plus Hot Spotter Analytics and text reminders versus usual care plus 12 sessions of PS-WHC over 12 weeks. The research will evaluate outcomes such as acute care utilization, health-related outcomes, patient activation, and engagement in supportive services.
