Description

Merced County is accepting proposals for FULL SERVICE PARTNERSHIP PROGRAM PROVIDER. The program aims to deliver high-intensity, individualized, recovery-focused, and age-appropriate behavioral health services to individuals with serious mental illness or serious emotional disturbance who are at risk of homelessness, hospitalization, or justice system involvement. Services include integrated mental health and substance use disorder treatment, coordination with primary care, housing support, peer support, 24/7 crisis intervention, and more, aligned with the Behavioral Health Services Act and Welfare and Institutions Code Section 5887.
