Change order to Service Contract CN11696 for the following: update item #15 “County Designated Representative” to Dr. Julie Kollar. Updated item #13 “Compensation” by increasing rates for SART On-Call and clarified language around SART Training Hourly Rate, as per attached revised scope of work.
