DSHS may cancel this Purchase Order without any obligation if goods or services are not received at shipping location by June 30, 2026. Service to DSHS office located at 12920 Totem Lake Blvd, NE Ste 215, Kirkland, WA 98034. Vendor is responsible for scheduling service at a mutually aggregable time/date with appropriate DSHS staff beforehand. Vendor will ensure the equipment and service listed in Everon CO-01/Job 301053679 functions as intended prior to leaving location. DSHS payment for services is contingent upon vendor's successful performance of deliverable(s) as stated above.
