Description

Telehealth Services for Low-Income and Uninsured Performance Period: November 25, 2024 - September 30, 2025 Provide primary and preventive care to under and uninsured Orange County population. Telehealth will include counseling, medication check, nutrition or diabetic education and appropriate acute visits. OUTCOMES: Outcomes shall be obtained from an electronic medical record database and provided to the County quarterly. Outcomes are due to the county by the 10th working day of the first month following the preceding quarter. At a minimum, quarterly outcomes will consist of: 1. Number of unduplicated patients with telehealth appointment 2. Number of scheduled telehealth appointments 3. Number of completed telehealth appointments 4. Patient satisfaction surveys 5. Improve depression or anxiety of patients through the use of telehealth counseling sessions 6. Improve the health of children through nutritional counseling. INVOICES: Quarterly invoices shall be submitted by the 10th day of the first month of the current quarter. Invoices shall reflect the date, contract number, covered period and amount. Each quarterly invoice shall be $22,070.00 totaling $88,280 for the service period. Payments shall commence upon contract execution.
