Advanced Care Planning Toolkit Package; Companion Educational Materials (4); Live Training Facilitation (3 sessions) Vendor will develop a toolkit and training for social workers and case managers who provide services to patients with dementia and their caregivers. The resultant materials will equip social workers and case managers with the knowledge, tools, and client-facing resources needed to confidently facilitate advance care planning conversations and support informed, values-based decision-making.
