A cluster randomized trial to evaluate the effectiveness and implementation of a standardized protocol strategy, SOCIAL HF, aimed at reducing disparities in access to heart transplants and ventricular assist devices by addressing perceptions influencing decision-making, subjectivity in social support and adherence evaluation, and poor group dynamics.
