Pest Control Service Agreement Date 5/21/26 One-Time Service Vacuuming and sanitization of false ceiling areas. Pest to be Controlled: Rats and Mice; Pest Control Service Agreement Date 5/21/26 Bi-Weekly Services Pest and rodent control service to be performed twice per month. General pest control to treat the interior and exterior of the main building, shed, and dumpster. Rodent control to check and re-bait traps and bait stations and set up and check monitors under all sinks in the interior. Pest To Be Controlled: Rats, Roaches, Crickets, Mice, Silverfish, Earwigs, Ants, and Spiders. Service dates: 06/05/26 to 05/04/2027
