This study aims to generate real-world evidence on the management of hypertension in older adults, focusing on the effects of treatment strategies—specifically, intensification or maintenance of antihypertensive therapy with or without de-escalation—on cognitive and cardiovascular outcomes. It will evaluate safety, including risks of injurious falls and adverse events, and consider patient-specific factors such as age, sex, frailty, comorbidities, and APOE ε4 carrier status. The research will leverage large datasets from the Veterans Health Administration and Intermountain Health, employing causal inference methods and natural language processing to inform clinical guidelines and improve patient-centered care.
