A project funded by the National Center for HIVAIDS, Viral Hepatitis, STD, and TB Prevention to use real-time prescription data to improve HIV care adherence in NYC by identifying and intervening with individuals who delay picking up ARV medications.

Description

This project develops, implements, and evaluates a collaborative “Data-to-Care Rx” model using pharmaceutical insurance claims data to identify persons with HIV in real-time who fail to pick up prescribed ARV medications by 30 days and 60 days. The project aims to implement targeted evidence-based interventions at each of these benchmarks to address adherence barriers and re-engage individuals in HIV medication therapy and primary care. The collaboration involves Amida Care Inc., Cicatelli Associates, Inc., the Brookdale Center for Healthy Aging at Hunter College, and the New York State Department of Health AIDS Institute.
