Description
This study aims to combine mobile health (mHealth), personalized coaching, and social work support to improve patient activation and clinical outcomes among kidney transplant recipients in socially vulnerable communities. The intervention leverages existing health IT to enable remote monitoring, enhance communication, and reduce health disparities. The project proposes a five-year, three-arm randomized controlled trial involving 339 adult kidney transplant recipients, comparing the efficacy of MyKidneyCoach and MyKidneyCoach+SW (social worker support) against usual care.
