EverOn Change Order Proposal 01 (Everon CO-01), job 301221467 scope of work: -Repair and Replace Door Strike at Door 314B -Includes labor and equipment; The vendor is responsible for accurate and successful delivery of all goods or services listed on this Purchase Order prior to June 30, 2026. DSHS may cancel this Purchase Order without any obligation if goods or services are not received at shipping location by June 30, 2026. Service to DSHS office located at 500 1st Ave S, Seattle WA 98104. Vendor is responsible for scheduling service at a mutually aggregable time/date with appropriate DSHS staff beforehand. Vendor will ensure door strike functions as intended prior to leaving location. DSHS payment for services is contingent upon vendor's successful performance of deliverable(s) as stated above.
