Evaluating the safety and efficacy of glucagon-like peptide-1 receptor agonists for pre-pregnancy weight management and improved pregnancy outcomes

A research project funded by NICHD to study the safety and efficacy of GLP-1 receptor agonists for weight management before pregnancy and their impact on pregnancy outcomes using electronic health records.

Description

This study will leverage the National Patient Centered Clinical Research Network (PCORnet) to identify a retrospective electronic health record (EHR)-based cohort of women with pregnancies and their infants to address important knowledge gaps related to GLP-1RA use and pregnancy outcomes. The hypotheses are: GLP-1RA use prior to pregnancy will reduce risks of obesity-related outcomes (e.g., gestational diabetes); Discontinuation of GLP-1RAs prior to or early in pregnancy may result in higher gestational weight gain (GWG); Use of GLP-1RA in early pregnancy may be associated with fetal/neonatal outcomes such as stillbirth, malformations, and NICU admission. The study will facilitate the longitudinal follow-up of a multi-center cohort, including weight/BMI, medication prescribing, encounters, diagnoses, procedures, and laboratory results, to inform clinical guidelines for GLP-1RA use among women who are pregnant or planning to become pregnant.
