Description
This study will implement and evaluate a multilevel intervention designed to strengthen social connectedness and improve adherence to endocrine therapy among breast cancer survivors. The intervention, CONNECT, combines Enhanced Monitoring with navigator-led education, social needs screening, and social prescribing to peer and community programs, grounded in Self-Determination Theory. 300 hormone receptor–positive breast cancer survivors will be randomized to Enhanced Monitoring or CONNECT. The study evaluates efficacy, implementation, and mechanisms of the intervention.
