This requisition is for K10 Vision, Inc., 1st year of 5 year agreement Year 1 Total Fixed Price Implementation Costs = $88,165 Year 2 Maintenance & Support & Hosting Service = $20,779 Year 3 Maintenance & Support & Hosting Service = $21,819 Year 4 Maintenance & Support & Hosting Service = $22,910 Year 5 Maintenance & Support & Hosting Service = $24,055. To be paid upon completion of services, DEP acceptance, and receipt of itemized invoice. Partial payments allowed. In the event that services rendered are not deemed satisfactory, the Contractor shall re-perform the work needed to ensure satisfactory service, at no additional cost to the Department. Failure to provide all services, or failure to provide services which are satisfactory, shall result in non-payment or termination of the Contract.
